STEP 3: Enter Your Personal Details like:
First Name, Middle Name, Last Name, Suffix (optional), Date of Birth (month, Day, Year), Email Address, Physical Street address, Apt, suite, Housing (own, Rent), His long have you been living there, Monthly Mortgage/Rent, Zip Code, City & State, Cell Phone, Home Phone Driver’s License Number or ID, Issuing State, Social Security Number(SSN), Main Source of Income, Monthly Income, other source of income monthly (optional)
